Description
Care Manager RNs sought to provide field based Care Management Services for White Plains area . RNs provide nursing and social work services for our elder clients according to the established plan of care. This position works within a multi disciplinary approach to care to provide comprehensive eldercare services for our clients.

Requirements
Appropriate professional license,
Must have at least two years of field based experience,
Experience with dementia care, psychiatric care and/or homecare a plus.
